Designation of Beneficiary 2007

Dear Member:

Upon your death, your Beneficiary(s) may be eligible for benefits depending on your rights with the Operating Engineers and Central Pension Fund at the time of your death.

Enclosed are the application forms necessary to change your Designation of Beneficiary. Your Beneficiary is the person(s) you designate to receive any benefits that may be payable at the time of your death. The forms must be fully completed following the instructions on the forms and should be sent to the Operating Engineers Office and/or the appropriate office according to benefit(s). You may name or change your beneficiary(s) at anytime by completing the application for Designation of Beneficiary and submitting to the appropriate office. An application for Designation of Beneficiary is only considered “filed” once it is received by the appropriate office according to the benefit(s).

Central Pension Fund: (Private Sector Only) Complete the Designation of Beneficiary Form including your signature, your witnesses’ signature and date. If you have been married one year (12 months) or more and have a vested interest, you need not complete this form, since the Plan of Benefits provides that your spouse will automatically be your beneficiary.

For a complete definition of beneficiary with The Central Pension Fund, refer to “A Guide to Your Benefits Booklet” explained in Sections 8, 9, and 13.

The Central Pension Fund, as explained in Sections 8, 9, and 13 of “A Guide to Your Benefits Booklet,” regardless of your designation, the qualified joint and survivor annuity will only be paid to your spouse. Furthermore, the qualified pre-retirement survivor annuity will only be paid to your qualified spouse, and the contingent annuitant benefit will only be paid to the contingent annuitant you have selected.

Local 501 Form: Complete the enclosed Designation of Beneficiary form, including your signature, your witnesses’ signature, and date. See Benefit Explanation, The IUOE Local 501, By-Laws, Article VI, Section 9, Local Union Death Benefit with Dues.

Operating Engineers Security Trust Fund: (Private Sector Only) Complete the enclosed Designation of Beneficiary form, including your signature and date. See Benefit Explanation, The Operating Engineers Local 501 Security Trust Fund, Summary Plan Description Booklet (SPD) of January 2006.

Basic Data Card: (Private Sector Only) In the section indicated, complete items 1 through 4, including your signature and the date.

Address Correction: If you move, you must send your change of address to the Union, the
Central Pension Fund and the Operating Engineers Trust Fund.

All forms must be fully completed. It is important that your Designation of Beneficiary be clearly identified so that there will be no question as to your meaning. Please follow the instructions on the forms.
